Il dolore dorsale e il dolore radiante alle gambe sono sintomi comuni.

Si eseguono spesso radiografie, generalmente in posizione eretta. Questo contribuisce a "classificare" il grado di scivolamento e determinare il tipo di spondilolistesi presente.
Nei giovani si raccomanda di evitare l'esposizione ai raggi X, quando possibile. L'attuale indagine preferita è una scansione RM. Questa viene generalmente eseguita con il paziente sdraiato, per cui lo scivolamento è talvolta meno evidente.
A volte, lo specialista potrebbe richiedere una scansione TAC.
